The New Support at Home Program – Rebuild Independence After a Health Setback
From the 1st of November 2025, the new Support at Home program introduced an intensive, short-term pathway of care to help frail older people regain their independence after a health setback. Unlike permanent in-home support, restorative care focuses on restoring capacity and promotes choice and independence.
The goal is to help rebuild daily abilities after a fall, illness or a hospital stay and learn how to manage new or changing health conditions to delay entry into higher level care. This can include clinical nursing, physiotherapy, along with regular monitoring, and can offer up to 16 weeks of care to help your person stay at home longer.
